Medical record information for the Class of 2019

Congratulations and welcome to Bucknell!

We are looking forward to your arrival to campus this Fall. In an effort to help make your transition easier and to help ensure a healthy campus community, Bucknell Student Health wishes to make you aware of some important information. You should be receiving a “Medical Record Form” in the mail around mid-May.  This form is to be completed by you and a healthcare provider and is required to be returned prior to your arrival on campus. It is important to note if you are a varsity athlete, you will need to complete two sets of forms…one for Athletics and one for Student Health.

Bucknell requires you to be up-to-date on your immunizations and to have a physical exam by a healthcare provider. The physical must be within one year of the start of classes which is August 24 this year. If you have had a physical exam within the one year time frame (since August 24, 2014), the Medical Record still must be completed and signed by the provider who performed your physical. If you have a physical scheduled for a time prior to when we expect the forms to be received by you, you may visit the Student Health webpage at www.bucknell.edu/StudentHealth to download and print the medical record form to take with you to your appointment. Currently, the form can be found by clicking Student Information and then Medical Forms.
