Why do we need a completed medical record? It’s simple, we require a complete medical record from all incoming students to help keep our campus healthy. The more we know about the health of our community members, the better we can serve any health needs. Also, ensuring all students entering our community are appropriately vaccinated against communicable diseases helps ensure the health and wellness of the entire campus community.
Because the completion of this form takes time and requires a signature from a health care provider, we recommend scheduling an appointment for a physical as soon as possible.
